You can text fields, text boxes, check boxes, and radio buttons to your subscription forms in order to request additional information from subscribers to your mailing lists. If necessary you can even create hidden fields within your subscription form. To add additional subscriber fields to your mailings click on List Settings and then Subscriber Fields on your left.

Use the drop down menu to select what kind of subscriber field you want to add. After you define what your subscriber field is for by naming it you can create a default value for it, choose whether to require your subscribers to fill it in before the form can be submitted, where on your form you want it to be appear(order), and any other lists you want this field to be available for(hold down ctrl to select multiple lists in the text box that appears on this page). Before saving your subscriber fields you have the option to preview or go back to your subscriber field list. Click on Add to save your new subscriber field. You can edit or delete your subscriber field on a list page that appears with all your subscriber fields. This list will also tell you what personalization tag your subscriber field has been assigned along with the type and order in which it appears on your subscription form.
